This web page is designed to give you the forms and documents you need to help you to implement your project successfully and complete the monitoring and evaluation requirements of your grant.

Confirmation of budget form
You should complete and sign two copies of both these documents, return one copy to us and keep the other for your reference.

Bank transfer forms

Please fill out the appropriate form and return it to us so that we can transfer payments into your account.

Monitoring and evaluation forms
Monthly Financial Monitoring Form
You should fill out a copy of this form every month of the projects duration, detailing every item of expenditure you make over £50. Completed forms should be sent to us with your Mid Term and End of Project Reports.

Mid Term Project Report Form
Use this form to submit your Mid Term project report if your project lasts for more than six months. Normally we will only make the second payment to your account when we have approved this report.

End of Project Report Form
Use this form to submit your End of Project report. Normally we will only make the final payment to your account when we have approved this report.

Using the UNISON logo
You should acknowledge support from the UIDF in all publicity and training materials used for the project. This might include leaflets, brochures, posters or advertisements. However, you should not use the logo in a way that would confuse your organisation with UNISON. Therefore you should not put the logo on your organisation’s letterhead, business cards, receipts or payment forms or other legal documents. Please find the UNISON logo in various formats below.
